What can I say? I used template of shield with eagle I've got on some folder on my hard drive with this kind of stuff. In fact I can put this file here if you want to see it (one on the left as you see): http://imgur.com/urALG5Q . I don't care what's a history of this file, maybe someone make it from the one you posted, maybe otherwise - in fact I don't give a fvck.

I take template make some stripes and put name of club in some fancy font on it, take the other template for "turkey shield", add 2 stars and that's all. I'm not the professional graphic, I don't draw it, I do it for fun and I use templates almost 99% of time, but if don't accept this kind of work you can DSQ me, no hard feelings.
